Create your account on our secure patient portal, then complete check-in before your visit. You can add your insurance card and e-sign your consent forms from your phone or computer.

Print the New Patient Packet below, fill it out at home and bring it with you. Or arrive 15 minutes early and we’ll have the forms ready for you at the front desk.

Appointments, cancellations, refills and medical records, plus insurance, copays and billing. Both are included in the packet.
What to bring
Photo ID and insurance card
We’ll make a copy of both at your first visit. Not sure we take your plan? See the plans we accept.
Your medications
A list of the medicines, vitamins and supplements you take, or bring the bottles with you.
For children
Your child’s insurance card and vaccine record. A parent or legal guardian should come to the first visit.
